Then, enter our parish code in the window under the line "Does your parish already have Formed." Our parish code is CC34WV. Click submit. Then a page will appear for you to create your profile. Follow the instruction. Remember you could create a new password, and don't have to use your email inbox password.

Here’s how easy it is to get FORMED: go to www.FORMED.org. You should see the registration box for parishioners. Now type in the parish code in the card you have received after mass. Now set up a username –
whatever username you’ll remember - and a password. You can keep it simple
now and change it later if you want. Now you are all set to use all the great
programs on FORMED.org anytime by simply logging in. It is that easy!
